Care coordinator

Clifton View Care Home requires a full time care coordinator

A minimum of 5 years’ experience working in Care homes is required.

The successful applicant will be required to oversee the day to day running of the floor, complete staff supervisions, ensure care plans, risk assessments, capacity assessments, deprivation of liberty referrals and relevant documentation is implemented and reviewed as required to reflect each residents current needs, triaging potential new residents, participating in MDT meetings.

The successful candidate will be working within a reablement/community bed setting working along side NHS professionals.

Experience in this or a similar role is essential.

Minimum level 3 diploma or degree in health and social care or equivalent is essential.

Ability to use Microsoft office competently is essential
